Payment for Services

I am also an out of network provider for most PPO insurance plans.

Many PPO plans will reimburse a portion of the cost of services. Prior to our initial session, it is recommended that you verify your out of network benefits for mental health services/behavioral health with a LCSW.  

If you utilize an out of network insurance (example Anthem Blue Cross) you are required

to pay full cost of the session at the beginning of the appointment.

At the end of each calendar month I will provide you with a billable receipt to submit to your insurance for reimbursement.
